How do we get a copy of our medical records?
You may request medical records by calling our office or submitting electronically.
Medical records are subject to a fee with the exception of the patient’s primary care physician or a referral from our office. A medical records release is required and you can fill out a release in our office or submit electronically. Medical records can take an average of 7-10 business days to have ready.
What do we need to bring to the first appointment?
A biological/adopted parent or legal guardian (with custody paperwork) will need to be present for the first appointment. Please complete the new patient registration electronically and submit before the appointment to prevent possible delays. If you are unable to complete it beforehand, please arrive 15 minutes prior to your appointment to complete it in the office. Please bring a valid photo id, insurance card for the patient, and a list of any medications that the patient is currently taking.
Where do we need to take our stool sample?
You will need to take your stool sample with the order provided by our office to the registration desk at the East Tennessee Children’s Hospital or your local hospital facility within 24 hours of collection. Please make sure that the container is labeled with the patient’s name and date of birth. It will not be accepted if it is missing this information.
What do we do if we need FMLA paperwork?
Please provide the office with the paperwork from your employer for the provider to complete. There is a $25 fee.
How do I make a payment or set up a payment plan for a bill?
You can pay your bill multiple ways:
- “Pay a Bill”
- Click the “Pay a Bill” button at the top of our website,
- In person in our office. We accept cash, check, Visa, MasterCard, Discover, American Express, and Apple Pay
- By phone with our office at 865-546-3998.
- By phone with our billing department at 865-670-6199.
- Our billing department will also be able to assist you with all billing questions including setting up payment plans.
Can my child come to their appointment without a parent/legal guardian?
For a return visit, an adult listed in the HIPAA section of the new patient registration form will have to be present until the patient turns 18.
What if I am going to be late to my child’s appointment?
Please call if you will be more than 15 minutes past your scheduled appointment time. Depending on the provider’s schedule for that day, if you are going to be more than 15 minutes late to the appointment you may be asked to reschedule.
Is there a patient portal?
Yes, please inquire at the front desk or call our office at 865-546-3998 to request access. An access email will be sent to your email on file.
Will I be able to see results on the patient portal?
Yes, most results will trend to the portal. Some testing that is faxed to our office may not automatically trend but we can send it manually upon request. If there is any testing that comes back that the provider feels like needs immediate attention, the nurse will contact you.
Are you closed for lunch?
The office will still be open to in-office patients but our phones will closed from 11:45-12:45.
